Jazz Yorkshire is now inviting nominations for people or groups who have made an exceptional contribution to the jazz scene in Yorkshire and the Humber region during 2011 - 2012.
Your nomination must be for people or groups who are based in and/or undertake the majority of their activities in the this region. See list of award categories on right.
In February 2012 the nominations will be shortlisted and the award winners will be chosen by a panel of judges drawn from across the jazz community including previous award winners. The awards will be presented at a special event at Dean Clough, Halifax on the evening on Monday 19 March 2012.

Please note this is not a POLL. The award shortlist and winners will be chosen by a panel of judges drawn from across the jazz community including previous award winners.

- You'll need to write no more than 150 words to let us know why you think your nomination should be the winner
- For all the categories, we are looking for who was involved, a clear explanation of why they did what they did and how successful it was
- You can make as many different nominations as you like
- All the information submitted will be available during judging and may also be used in promotional material in connection with Jazz Yorkshire
